An established NDIS provider (operating for 2+ years with a strong client base of 100+) is seeking a motivated Level 2 & 3 Support Coordinator with a minimum of one year experience to join their growing team.

✨ About the Role
We are looking for a Level 2 & 3 Support Coordinator who is confident, proactive, and passionate about supporting participants to reach their goals.
What You’ll Be Doing
- Assess the needs of clients and develop individualised support plans aligned with their goals and preferences
- Coordinate and monitor the delivery of support services, ensuring high-quality care is consistently provided
- Communicate effectively with clients, families, and service providers to ensure seamless service delivery
- Advocate for clients’ rights and ensure they have access to the appropriate supports and resources
- Maintain accurate client records, progress notes, and documentation in line with policies and NDIS requirements
- Collaborate with the broader team to strengthen service delivery and drive positive client outcomes
- Work autonomously while contributing to a supportive and well-structured organisational environment
🔍 What We’re Looking For
- Relevant tertiary qualification (Social Work, Disability Studies, Community Services, or similar)
- Minimum 1 year experience in Support Coordination or case management
- Strong understanding of the disability sector, NDIS legislation, policies, and funding structures
- Excellent communication and interpersonal skills, with the ability to build trust and rapport
- Strong problem-solving skills and the ability to manage complex situations with confidence
- Passionate about improving the lives of people with disabilities
- Comfortable working in a hybrid role (remote + community visits)
- Able to work both independently and collaboratively
